Our establishment offers Neurotoxins and Fillers from renowned companies such as Merz, Galderma, and Allergan. To ensure compliance, previous certification is required. For  Allergan and Galderma an office space is also required.  Familiarize yourself with the products offered by both vendors to appreciate the passion in aesthetics.

Merz can ship to a residence or business space.

Allergan/Galderma requires a business space in order to ship to, they need for your name to be on the lease or one of the following:

  1. Full lease agreement with full business name, shipping address & signed by the Lessor and Lessee.
  2.  Utility Bill with full business name & shipping address.
  3. Invoice with full business name and shipping address (purchase order, moving invoice, business taxes, etc.)
  4. Picture of outside of building that includes the business name, building number and/or suite number to confirm the shipping address

Approved Certification for Neurotoxins is required before purchasing the Confidence Kit. The Confidence Kit is neurotoxin continued education.
 
The confidence kit includes Merz Xeomin Bootcamp training and it is a two part class. The first part is a 2 hour virtual bootcamp regarding the product Xeomin. The second part is a two hour virtual training with Jen Holt, where they will inject 2 patients. 
 
*Please Advise: After purchasing the confidence kit and successfully receiving your initial vial order please email inventory@hydreight.com to request setting up Xeomin Bootcamp training. Once you have completed your two hour course on Xeomin please email inventory@hydreight.com to schedule your 1-on-1 training with our Merz representative Jenn Holt.

  4. NOTE: BA's need to follow Rules and Regulations from the state board for their license to offer aesthetics.
  5.  Some boards require proximity and others do not allow registered nurses to offer aesthetics. Some states require the BA to be a nurse practitioner. For example: Florida requires BA's to be a nurse practitioner to offer aesthetics. 

Once we have approved your certifcations for neurotoxin/fillers please submit a tech support ticket to add aesthetics to your menu.

Yes you can. Advise the patient to let the item thaw and come back to room temperature. As long as there's no precipitation post-thaw, the item won't be damaged. 

If the item does show precipitation, please send a photo of the vial with the precipitation to inventory@hydreight.com and indicate which patient this is for.

Pharmacy Name Excursion Statement
Seven Cells Seven cells testing shows the medication (GLP1s) is stable for 28 days in 86F degrees can tolerate upto 140F for 2-3 days without degradation. We do recommend refrigerating the the medication as soon as you receive it before injecting
Perfect Rx ProRx, who manufactures the GLP-1's we provide has done excursion tests that show the medication still being stable after 72 hours without any ice included. We have also done excursion test in all seasons to all areas of the country to ensure our packaging keeps the medication remains within the acceptable temperature range. We understand that the customers are accustomed to receiving these types of medications cold, but the greater concern with these is freezing. As stated, the medications are very heat resistant, but if it get's to a temperature where it would freeze, that immediately ruins the medication and it will not be effective. We have also done tests that include extra ice packs, and that showed that after just a couple hours, temperatures were low enough and we found some vials had frozen.
   
Empower Semaglutide/ Tirzepatide Shipping Change
REVISED JULY 26th, 2024

According to data collected in internal studies conducted by Empower Pharmacy, Semaglutide and Tirzepatide injections stay stable for up to seven days at temperatures up to 104°F. To ensure the preservation of product, vials should be transferred to refrigerated conditions (36–46°F) upon receipt.
TMC NAD+ Injection is stable for at least
24 hours up to 104°F, 48 hours up to 86°F, 72 hours up to 77°F and for at least 90 days at refrigerated
conditions.
With Thermosafe cooler, allowable time without impacting quality is 51 hours with outside temp upto 104F, 86hrs with outside temp upto 86F, 120hrs with outside temp upto 77F


Sermorelin/Glycine Injection is stablefor at least 90 days at refrigerated conditions.

SEMAGLUTIDE
If your Semaglutide Injectable medication was left in the heat for 7 days, you can refrigerate it immediately and continue using it for the duration of your treatment. The effectiveness* is not reduced as long as it is refrigerated and used within the prescribed period.

¹ In a controlled analytical study, Semaglutide injectable underwent testing under an extreme temperature (120°F), with potency maintained.

TIRZEPATIDE
If your Tirzepatide Injectable medication was left out in room temperature, you can refrigerate it and continue using it for the duration of your treatment. The effectiveness* is not reduced as long as it is refrigerated and used within the prescribed period. Studies are currently being conducted for Tirzepatide Injectables under extreme heat conditions.

¹ In a controlled analytical study, Tirzepatide injectable was stored in room temperature for 7 consecutive days and maintained potency for 90 days when refrigerated.

Would recommend ordering preserved versus preservative free. Preservative free has to be used within 4 to 24 hours of puncturing. And preserved typically has 28 days after you puncture the vial. There will be a beyond use date stated on your vial or an expiration date meaning that you shouldn’t use it after that date or the 28 days, whichever comes first. It will save you money to get a preserved product. And you’ll have a greater profit margin as well. And unless a patient shows an actual reaction to a preservative, the main one is benzyl alcohol, then there’s no reason to order preservative free. If you do end up with a patient that needs preservative free I recommend advising them on the cost difference and having them cover the cost for you.

Benzyl alcohol is most common, with very few reactions.

Without a sterile hood- we follow the 3-2-1 rule (this is my way of remembering the USP 797 low risk compounding guidelines). 3 ingredients in the bag max (including the fluid), 2 bag sticks to get those ingredients into the bag, 1 hour to use the bag on a client. This keeps you at the lowest risk possible for any issues with bacterial growth, contamination etc.

Different therapies vary, but in general not more than one IV per "therapy" per week. I would not likely want to give more than 2 in a week of varying therapies either but there are always some exceptions. For example Hydration up to 3 times a week if someone is working outside in 115 degree heat might be warranted. Typically though 1-2 per week and probably not back to back days either. More 2-3 days apart or so.

There is a protocol for adding the medications - ondansetron, ketorolac, famotidine to any drip as long as an intake is completed and a medical prescriber approves dosing. 
 
There are 3 types of add ons. 
  1. Those that are written in the IV DRIP, or IV PUSH protocol as suggestions to allow you to tailor more to your client needs. 
    1. Example: In the Empower Athlete Recovery you can add on a slow push of Bplex. 
  2. Those that are in the "add ons" section on the portal-
    1. Standalone IMs- Glutathione, Bplex, B12 IM - can be added to any drip. 
    2. Medications- ondansetron, ketorolac, famotidine can be added to any drip as long as an intake is completed and a medical prescriber approves dosing. 
  3. Site specific add ons- Drips, IMs, Pushes

Sorry, no. The multi-dose bottles do not come in smaller sizes and it would be out of compliance to try to separate them. The vials themselves are sterilized at the pharmacy, these products are compounded in ​a USP 797 or 800 environment, one you cannot replicate. The testing for BUD (Beyond use dating) and sterility is based on testing done by third parties for each formula and batch respectively. Manipulating is unsafe. 

What about sulfa allergies and glutathione or B1 or methionine?

The patient would need to have a sulfur/sulfite allergy to have issues with B1 and actual SULFUR the
element to have any issues with glutathione or methionine. While Sulfur itself may be a part of the
chemical structure of sulfates, sulfites and sulfa drugs- it does not have cross reactivity or reactions as it
is a crucial element in the human body.

sulfuric acid and are present in many medicines, supplements, and personal care products – they are

So we have

1.Sulfur- allergy needed to be reactive to Glutathione, B1 (thiamin)and/or methionine- this allergy is non existant-Sulfur is a chemical element is the third most abundant mineral in the human body. Sulfur is also found in many foods. While some individuals may be allergic to the action of sulfonamide antimicrobials and sulfites, they do not have true allergy to sulfur itself. True allergy to sulfur is virtually impossible, as sulfur is present in all living tissues and humans cannot survive without it.

 

 

 

 

 

2. Sulfa- allergy to antibiotics not related to the above-Though 3 to 4% of patients claim to allergic
to sulfa drugs, only 3% of these patients have a true allergy to sulfonamide antimicrobials.

3. Sulfite-allergy needed for B1 (thiamin)- Sulfites are present in many forms including bisulfite,
metabisulfite, and sulfur dioxide. Sulfites cause allergy in approximately 1% of population.

4. Sulfates are widely present in everyday life: sulfate-containing drugs, supplements, and personal
care products. Sulfate allergies are rare

 

2 times a week but ask yourself is this normal, why is this patient dehydrated so often? Should they really see a PCP?

No, they need to be 3-4 days apart

It depends on the exact products are you looking to infuse. The short answer is no- these drugs have to be prescribed by and infused by healthcare provider who bills Medicare part B unless dispensed thru the patient’s insurance by a pharmacy and then it is part D and we are not an insurance billing business provider. They are also extremely expensive.

We could create an iron support IV Infusion drip. Site specific drip development based on what products you can obtain are $300. 

For more detail on iron read on... Low iron is a complex medical issue that can result from many different processes in the body. It is usually discovered through extensive work up with the client's medical team which often is a primary care provider and sometimes also a specialist. 

Due to the many various reasons for low iron, helping someone with low iron is very patient specific. 

For example, if it is related to a GI issues (i.e. GI bleeding, chronic GI condition that results in malabsorption) vitamins such as Vitamin C, Folic Acid and B12 will be beneficial in helping their body produce red blood cells. 

Vitamin C in general is helpful when patients take oral iron, in helping the body absorb the iron from the oral supplement. Therefore, Vitamin C, is helpful to all clients who may have iron deficiency. 

B vitamins are also, in general, very helpful for our body to create new red blood cells. 

Given the increased demand for offerings that may help iron levels, we could consider what drip formulation makes the most sense and eventually creating a drip especially with this population in mind, but at the end of the day, it will also be very important for the client to also be getting iron into their body somehow, whether it be through diet, or oral supplement or an IV iron infusion. We will not consider IV infusions at the Hydreight level as it is a prescription, the Iron medications are quite expensive, and insurance often helps to cover this service.

SDV are similar to the preservative free ones that have to be used within 4-24 hours. A SDV created by a pharmacy would exponentially increase the cost to the BP and to the client. This is not a scalable marketable item for compounding 503B pharmacies.

You may dilute your pushes with NS or LR. Normally a 1-1 ratio is used.

Q: Is there an age limit? If not, is there a legal document that parents can sign for approval of minors?
 
A: We prefer 18 and up. All ADULT patients require and MSE prior to receiving their initial drip, if their medical history changes or every 6 months whichever is soonest. Some states require a consult every time and it is up to you to know your state requirements. 
 
IF you have a minor then the following is REQUIRED.
 
  1. The age range is 15-17
  2. GFE/MSE every time this is non negotiable for everyone's sake
  3. Parent must be present every time a service is performed
  4. Parental Consent and Signauture is required every time a service is performed.
  5. Advise fluids only but the PRESCRIBER will decide every time if the minor is eligible for any ingredients and what doses. I have advised the prescribers not more than 200mg of ascorbic, yes only 200mg NOT 2000mg and 3mg or less of zinc. 

AESTHETICS is for Adults only. Patient must be 18 or older. 

The individual business partner will have to reach out to the pharmaceutical vendor directly that supplied the B12 with the lot number. The reason for this is that the pharmacy will have to check their MSDS sheets and may have to reach out to all the suppliers of ingredients in the vial. The B12 itself, the preservative, solution, and any other ingredient they use to balance pH, etc.

In addition, it can't be a one-time outreach because sometimes the supplier of the powder or ingredients change then another outreach may need to be made. You will not be able to tell by the label on the vial if the suppliers have changed. Each lot number could have different suppliers for the same ingredients. The ultimate answer is that, while we cannot absolutely guarantee there is no alumimum, there is not a measurable amount. And you may still happily reach out to the pharmacy who made your B12.

“Cyanocobalamin is converted to hydroxocobalamin is converted to the active/bioavailable form - Methylcobalamin. Methyl is the preferred form. If you do not have access to Methyl, please proceed with hydroxocobalamin weekly IM and discuss getting methylfolate or methylcobalamin oral 3 times a week. 
 
Weekly is the maximum to give. Your standing order must be followed. Please do not exceed 1000mcg per week. 
 
IM is the preferred route of delivery due to the slower absorption over time for your body to use the vitamin. IV will cause a quicker peak and then there will be a drop off. Think of this as a hill vs a mountain." - Dr. Urbatchka

For High dose vitamin C (HDIVC)- You will need a hood. It requires 4 ingredients which is more than our low risk 3-2-1 rule.

Then comes all the questions with the hood-

What type of hood are you getting? What ISO Class? Will you be ventilating to the outside? Are you getting a clean room? Will you be garbing and gowning head to toe? How often are you testing your hood? How often does your state require testing? How often do they require you to do your glove fingertip test? Velocity measurements? HEPA filter tests? Air tests? Temp/humidity? and so on and so forth. Want to prep you as best I can😎

I would advise researching your state laws for brick and mortar as well.

Do you need an inspection by the BOP or any other board? What does the Nursing board say? Who's got jurisdiction over the location of the hood and when they visit what do they expect from you? Do you need PNPs? or SOPs? On the hood and mixing and testing, anything you are performing or on what to do if there is an incident etc.......

Ultimately, you may want to hire a pharmacy/pharmacist/sterile compound lawyer to check all your boxes. 

Once you have the hood sorted out then we can talk about HDIVC. Education on HDIVC and how it causes shifts in electrolytes and a high amount of sodium and how at 15gm and above it becomes oxidative which is how it helps with cancer, acute infections, wounds etc. 

We need to have training on the contraindications, the labs that have to be done prior, then the telehealth with Dr. P to ok the patient and their frequency and dosing. You will need to be trained on administering, monitoring and step dosing. 

Then you will administer and you monitor blood glucose before and after. 

Currently- The highest we can go without all the above is 7.5gm. AND That is anti-oxidative not oxidative (which supports cancer) which is the purpose of high dose.

 No. Syringes are actually not designed to store medication at all. There was a recall back in 2015/16 ish where the DEA actually went around to every pharmacy that bought certain lots of syringes from BD and either confiscated or documented if they had been used. Medications may not maintain potency, and the rubber may lech into the product. There is no way we can safely support such practice and to do so would be out of compliance with Hydreight and normal standards.

Yes, these are level 2 consult. Unless you are having an event and you submit a ticket to notify Hydreight of the event for proper staffing, then Vit B12 would require only a quick Level 4 consult ($6). Please note the Hydreight team will have to set this up for you.

In order to assist it is best to wait to respond as it is pertinent you communicate the correct information around your inquiry.  As a nurse on the Hydreight platform, you are an independent contractor of the medical practice Healthcare Prosoft, not an employee. It’s crucial to cooperate with the investigators and answer the questions with accurate information. As a reminder, all consents, services, and orders are documented and stored in the app. The prescriber must conduct a medical screening exam, and provide the patient’s order for the RN. The RN then executes the protocol pursuant to the prescriber’s authorization.  We recommend you seek legal advice anytime you are contacted by a professional board (ie Board of Nursing, Board of Pharmacy, Board of Medicine).

Chemotherapy is oxidative. When a patient is on undergoing chemotherapy, we must avoid anti-oxidants, such as low dose ascorbic acid (anything Hydreight dose is not high dose) and glutathione, certain B vitamins etc. When on chemotherapy - we can support patients with fluids, magnesium to help with the pain. When the patient is not on their chemotherapy- about a week off or away they may have ALA- all 3 ALA options are currently are site specific and require an order from Dr. Lindsey info@wellnesspharmdconsulting.com. The robust option is the best, and requires labs, consult and training. Another option, when the patient is off their chemo or about 1 time per month is a Myer's. Because while we recognize they are being depleted, and do not want to interfere with their chemo, we still want to support the wipe out of their nutrients and systems. We always recommend they discuss with their oncology team and Hydreight does require an exam for new and medical history changes. Any adverse event, or reaction, or side effect or abnormal reaction needs to be documented as an incident. Please fill out an incident report within the app and if necessary, ask Dr. Lindsey for a word doc if you run out of character space in the app. ALWAYS INCLUDE your LOT NUMBERS, BUD and which PHARMACY compounded the products used. We need all medications listed, even if the adverse reaction may not have been related to the products themselves.

1. Standalone Intramuscular (IM) Injections:
  • You’ll find standalone orders for the following IM injections under the “Add-Ons” section:
    • B12
    • B Complex
    • Glutathione
    • Topical Lidocaine (for numbing)
  • These can be given independently (without a drip) or paired with a drip if the client opts for one.
2. Drip-Specific Add-Ons:
  • For each drip, there are specific add-ons listed in the signed protocols. These are tailored to complement the drip:
    • Example: The Olympia Immunity Drip has an add-on for the Tri-Immune IM injection.
      • If you’re administering the Immunity Drip, you can also give the Tri-Immune injection.
      • If you only want to give the Tri-Immune injection (without a drip), a site-specific order is required.
    • It is highly recommended to include an average cost of add ons in your drip prices because the add ons in the individual signed protocols aren't to be added to any drip, only the ones for the specific drip they are listed. 
      • Example: The Empower Detox Hangover has add ons of Glutathione push and tralement and B12 
        • if you added the Tralement you would have to omit either ascorbic acid or mag from the bag to keep to the 321 rule. 
        • If you wanted to add on a glutathione or B12 push then that is allowed bc it is on the signed protocol. 
        • You could not add on zinc as it is not on the signed protocol.
3. IM injections paired with drips- For example: MIC-Containing IM Injections:
  • These injections are paired with specific drips. However, to administer them as standalone injections, a site-specific order is necessary.
  • NOTE: You can contact Dr. Lindsey for any creation- IM/Drip/PUSH- you want to add to your business as site specific. The cost for adding is $125 for IMs and $300 for Drips/PUSH
4. Medications (Ketorolac, Ondansetron, Famotidine):
  • These medications can be added to any drip, provided:
    • The client completes the intake/screening tool.
    • A prescriber conducts a consult and approves the medication.

 

You could, but it is not necessary.  Omission is always allowed, not addition. EXPLICITLY FOLLOW your signed protocol. Price of your service should reflect the amount of ingredients you are using. 

As long as the strengths match the product on the protocols, you may use them interchangeably. Examples are glutathione 200mg/ml, ascorbic acid 500mg/ml.

The answer is YES! Whilst we provide custom mobile compliant menus created by our Medical Team. BPs are not bound to use only those. You may contract the services of Dr. Lindsey Urbatchka and Dr. Palumbo at an additional cost and create an approved Menu. There is an ongoing list of items or you and our medical team can get creative! 
 
Vitamin D injections are a site-specific protocol and require labs done within 30 days. Hydreight anticipates BPs will be able draw these labs at some point. If the patient has their own lab level that works too. But you have to have a lab level to execute the protocol. This site-specific protocol also comes with an attestation for patients regarding additional oral Vit D supplements as we do not want to cause an overdose. The cost is $125 and includes the protocol and attestation.

There are multiple strengths out there. Please reach out to your Medical Team if you want to double check your math and give 1000mcg.

You can order from other supply vendors if it’s the same strength just follow whatever protocol it's being used in whether it be in the Empower or Olympia.

The menus were formulated so that if one pharmacy did not have product, a Business Administrator (BA) would not have to omit product from their protocol. That being said, it is pertinent to follow the protocols explicitly. This is for compliance.

If you have the same strength of product listed in the protocol then you may use it. For example, Ascorbic acid is 500mg/ml from multiple suppliers and so multiple suppliers could be used to fulfill the protocol. Another example is you may use glutathione
from Olympia on the empower protocols because the strengths match. The B complex is identical between Olympia and empower in strengths and B vitamins so that may be used for either set of orders.

It is important to make sure you adjust accordingly to the strengths.

In some states, certain services must be provided by licensed health care professionals practicing within a medical practice owned by a licensed health care provider.  We call these states the “corporate practice” states because they have certain laws that prohibit unlicensed individuals from owning and operating medical practices.  Business partners operating and health care professionals providing services in these corporate practice states are independent contractors of Hydreight’s friendly PC network.  Consequently, the services offered by these partners and providers are performed on behalf of a medical practice owned by a licensed physician as required under that state’s laws.

Certain services must be administered by a licensed medical professional.  Whether a service like Botox, fillers, or other cosmetic procedures, they must be administered by a licensed medical professional varies from state to state. Business partners should consult their medical director for oversight and contact their local state medical or nursing board or a health care attorney in their state to determine what activities constitute medical or nursing services under their state’s laws and whether there are any additional requirements for delivering such service (for example, a telemedicine consult prior to administering the service). 

 
 OH Board of Pharmacy requires a TDDD License. Approved credentials for a responsible person for an IV Therapy clinic can be a "DO, MD, APRN, PA, or RPH." Their regulations state that "a responsible person must be physically present at the location for a sufficient amount of time to provide supervision and control of dangerous drugs on-site." Ohio Admin. Code 4729:5-2-01. The number of hours the board requires that person onsite would be something on your end to determine with your Responsible person and apply for the TDDD license for a physical address location.

OH Board of Pharmacy Compliance Specialist shared info on the TDDD application process:
“If the facility is strictly IV hydration, then it would be category of clinic with sub-therapy of IV therapy.  If there will not be scheduled controlled substances, then a category 2 is fine. The category 3 fee is more than the 2 but allows controls. The application is required to be submitted through the portal; anyone can do it for the TDDD. A word of caution, whichever email is entered as the contact email will receive all notifications including renewals. I cannot tell you how quickly the application will be processed by licensing, I have no idea what their workload is, additionally, if there is an issue with the application, such as additional information required, it will lengthen the time. After the licensing team processes, the application, an inspection will be required by a Compliance & Enforcement which we generally complete within 30 days.”
The short answer is no. As long as you follow the immediate use standards set forth, 2 ingredients plus the bag=3 ingredients, 2 bag sticks, and Hydreights requirement of 1 hour to use the bag then you are within the USP 797 immediate use guidelines. The use of a hood applies to CSP (Compound sterile preparation) 1, 2 and 3 categories.

Section 1.3 Immediate-Use CSPs When all of the following conditions are met, compounding of CSPs for direct and immediate administration is not subject to the requirements for Category 1, Category 2, or Category 3 CSPs: 

 

  1. Aseptic techniques, processes, and procedures are followed, and written SOPs are in place to minimize the potential for contact with nonsterile surfaces, introduction of particulate matter or biological fluids, and mix-ups with other conventionally manufactured products or CSPs. 
     
  2. Personnel are trained and demonstrate competency in aseptic processes as they relate to assigned tasks and the facility’s SOPs. 
     
  3. The preparation is performed in accordance with evidence-based information for physical and chemical compatibility of the drugs (e.g., approved labeling, stability and compatibility studies). Covered by our PharmD
     
  4. The preparation involves not more than 3 different sterile products. 2 ingredients + bag = 3, updates coming prior to 11.1.23
     
  5. Any unused starting component from a single-dose container must be discarded after preparation is complete. Single-dose containers must not be used for more than one patient. 
     
  6. Administration begins within 4 h following the start of preparation. If administration has not begun within 4 h following the start of preparation, it must be promptly, appropriately, and safely discarded. Hydreight sticks to 1 hour on this rule
     
  7. Unless directly administered by the person who prepared it or administration is witnessed by the preparer, the CSP must be labeled with the:
    1. names and amounts of all active ingredients,
    2.  the name or initials of the person who prepared the preparation
    3.  the 4-h time period within which administration must begin. This is 1 hour at Hydreight

HIGHLIGHTS OF ANNOUNCEMENT:
Effective September 1, 2025, all elective IV therapy services offered outside traditional healthcare settings in Texas must comply with the new law. This means:
  • Every elective IV therapy session must have a consult and an order must be issued by a physician, PA, or APRN under a valid prescriptive authority agreement and adequate physician supervision. 
  • Only physicians, PAs, APRNs, or RNs may administer elective IV therapy, and only under adequate physician supervision.
  • Supervising physicians are responsible for ensuring that all delegated providers are properly qualified and that protocols and standard operating procedures are in place.

To begin the process for Anazao you will need to locate a medical office and a MD,NP, or PA license that is affiliated with the office location. The office location must be medical related normally a dental office or a chiropractor is NOT considered a medical office. The MD/NP/PA does not need to be present at time of delivery. 

Reach out to friends, family, co-workers, or peers to see if anyone has a medical office and a MD/NP/PA license to ship to. You want to ensure you make a safe connection with someone who can store your vitamins and allow you access for pickup. 

Once you locate a medical office and a MD/PA/NP license please email inventory@hydreight.com with the information and we will send it to Anazao for approval. It can take up to a week for approval and they may ask for additional information for final approval.

Please know that this is in no way all-inclusive and there are many other steps in the process of isolating a vitamin or mineral in mass and then taking it from that powder form to a patient's body. Simply, not everything is derived from living matter aka organic. Vitamins are typically (b vitamins). But Minerals are not (magnesium).  Minerals can be isolated by chemical reaction. Here's some background:

  1.  The pharmacies that compound your products are required to undergo state board inspection and most have undergone at least one FDA inspection. This ensures cGMP (current good manufacturing practice) are being followed.  
  2. As for sourcing the ingredients come from suppliers such as pcca, medisca, letco, etc which are also required to undergo FDA inspection. 
         a) These suppliers source from multiple locations depending on the product world wide. 
         b) They have a certificate of analysis for each powder and and MSDS sheet (if necessary). 
              i) Meaning each powder has under gone testing for purity etc. 
             ii) Some powders have activity that has to be taken into account. 
            iii) Other powders are so potent that when a pharmacy receives them they must be aliquoted and then sent for a new testing in order to measure amounts that are safe for human consumption.
  3. The suppliers have their own MSDS and certificates of analysis done on the products they source and are sent with the products to the pharmacy.
  4. The pharmacy then compounds your product according to a recipe. After it is compounded, they send the product for their own testing for potency and sterility to a third-party lab (eagle analytical for example). 
      a) This sheet is kept on file at the pharmacy. 
      b) The batch cannot be released unless testing is passed.
  5. Preservatives are required for beyond use dating to be more than 4-24 hours after puncturing a vial. The most common preservative is benzoyl alcohol. It RARELY causes any reaction. In fact, I haven't seen one yet. But never say never.
  6. Most of these products have a variable shelf life 30-180 days from the date they are compounded. 90 is very common. 
      a) Beyond use dates are set by the pharmacies thru the third party testing company. They will pay to have each individual formula tested at 30, 60, 90 days etc.... to see if it holds potency greater than 95%. 
    Now, for the commercially available products that are not compounded a similar more rigorous process occurs. Powders of the said vitamin or mineral or substance are purchased in bulk, they are pharmaceutically manufactured in bulk in a lab, sometimes mixed together in a specific combo, made into an injection, tablet, capsule, etc..... and they may have more than just benzyl alcohol as a preservative (although it is still pretty popular). The injectables do not typically have any coloring but as you know tablets do.  They are required to go through FDA approved manufacturers and these products have actually gone through the FDA approval process. Meaning a pharmaceutical company paid at least 100s of thousands to patent their product for 20 years. It takes about 13-14 years to get through all the studies and onto the market. These products will typically have a longer shelf life from when they are made most are at least a year if not more. 
     Additionally, Chemical reactions may be used to isolate an ingredient (mineral typically), the mineral must be present to isolate it, for example you can't get magnesium from something that doesn't have magnesium. You can mix magnesium sulfate with hydrochloride and make magnesium chloride..... 
     The ingredients are pure unless they have to be diluted due to potency, like thyroid meds-levothyroxine and liothyronine, those are so potent we must dilute with something like microcrystalline cellulose to prevent an overdose that could kill someone. 
    Without asking every pharmacy on every chemical for their C of A's (cert of analysis') and MSDS sheets we will not be able to exactly answer which are derived from chemical reactions and which are extracted from plants/animals. And they do not HAVE to share that information with us. The most important piece for compounding something sterile that is going to be put into a vein, is that it is tested for purity, and after compounding sterility and potency. All of these boxes are to be checked before release for sale. We may have a recall or 2 down the line from the pharmacy. We have PNPs in place to manage this. This is not common but does happen, even with FDA approved products.

Yes, keep a log. Check your vials, they will say how they need to be stored and be sure to read them when you get them. Refrigerator Temperatures: 2-8 degrees celsius (36F to 46F)
Room Temperatures: 20-25 degrees celsius (68F-77F) depending on the pharmacy, they will allow excursions from 59F-86F degrees Fahrenheit for room temperature. 

Storage for cold sensitive items should be stored in a non-food refrigerator. A locked refrigertor is recommended to ensure vitamins are stored safely, securely, and to avoid tampering. 

If you are traveling with refrigerated items- take a cooler, or a refrigerator with a thermometer to track your temperature so that you do not let your meds go on excursions outside the pharmacy’s required temperatures.

Follow the instructions on the vial when you receive the product. Pharmacies can vary in their storage requirements and may also update or change them so always read the vial. Use a fridge that is only used for your vitamins.

They are as similar as they can be using the ingredients that are available at the pharmacy. For example, Biotin is 10mg/ml for Empower and 0.5mg/ml for Olympia. You may interchange the two brands, but pay attention to the dosage strength.

Half-life.  It will oxidize if it is added to other nutrients and then you won't get the reduced form in the patient's body which is what it needs to use in the metabolic processes. 

These are fat soluble vitamins and incompatible in a water-soluble solution such as NS or LR. They must be injected into a muscle/fat for proper absorption. They also accumulate in the body and in the case of Vitamin D, an overdose can be toxic, which is why education and labs are required for D3.

Beyond Use Date (BUD) is very different from expiration date. USP Chapter 797 defines BUD as the date or time after which a compounded sterile preparation (CSP) may not be stored or transported and is calculated from the date or time of compounding.The Beyond-Use Date is crucial in ensuring the safety and effectiveness of the medication, especially for compounded drugs where multiple ingredients are mixed to create a customized medication for a specific patient. It helps prevent the use of medications that may have deteriorated or become unsafe over time. The determination of the Beyond-Use Date takes into account factors such as the stability of the drug, storage conditions, and the specific formulation used.

 Then in order to fulfill a B12 order you would need to get hydroxocobalamin or cyanocobalamin. In your body cyano is converted to hydroxo and then to methylcobalamin. While some of the population has some form of a methylation mutation or deficiency, most of the population can convert and methylate at least some of the product. Furthermore, there are varying opinions each B12 has it's lane. Methyl may be more suited for mental health and hydroxo more for free radical removal. Please know that as long as you are getting a patient B12, you are helping them. Cyanocobalamin is not poisonous.

The only methylated vitamin available in the US is methylcobalamin. 

All of these products work differently and help assist the body in energy, metabolism and overall health. Methionine may aid athletic performance by supporting bone strength and it's role in creatine synthesis. It acts as an antioxidant. It helps with metabolism and absorption of essential minerals. Inositol can help lower risk of metabolic syndrome by increasing insulin sensitivity, improving blood sugar, blood pressure and helping lower triglycerides. Assist in lowering cholesterol. It may help improve serotonin and dopamine as well. Choline can enhance muscle performance during exercise and improve stamina through its activity at a cellular level. Choline supports communication with muscle fibers and promotes muscle recovery following repetitive motion, resulting in better overall training output.

 Normal Saline is 0.9% Sodium Chloride while Lactated Ringer's is a solution including mineral salts- sodium chloride, sodium lactate, potassium chloride, calcium chloride. Normal Saline is required when diluting glutathione and mixing NAD. LR is not allowed for these 2 products. Always follow the orders and protocols, which typically will give you a range 500-1000ml of fluids NS or LR in order to ensure you have options especially during times of backorder.

 Vitamins and Medications are good for 28 days after puncturing unless specified otherwise in the item description and be sure to check the Best Use Date listed as well. Labels are available for purchase on the ordering portal:

People with the MTHFR gene mutation methylate 25 to 75% of the time. It depends on which allele they have. They do not need always methylated form, as their body will still continue to methylate partially. The only methylated vitamin we provide in the United States that is an injectable is methylcobalamin.
 
Please note cyanocobalamin is not cyanide. It is not poisonous and it can be given to patients that have this mutation. Same with hydroxocobalamin.
 
In fact, some patients do better with hydroxy then they do with methyl, the reason for this is if you over methylate someone who hasn’t been methylating very well. You actually caused them to feel worse. 
 
Patients with this genetic mutation, still methyl. They need to be taking oral vitamins that contain B6, B12, and B9.

DOSAGE AND ADMINISTRATION: Avoid using the intravenous route. Use of this product intravenously will result in almost all of the vitamin being lost in the urine.


Since it is not labeled as such but the risk to the patient is only lack of absorption you have some choices. 
 
  1. Follow the package insert. Omit the B12 from the drips and offer it in an IM shot at this time. 
  2. Mix B12 into a bag and test it on one of your staff- while we do not believe it will cause any harm, we do not know if the lower pH (4.5) listed on the vial will cause any burning or tingling.
